This is where playability/fudge factor has to come into it, as a DM & player the last thing you want is one player for what ever sat with his thumb stuck up his bum doing nothing in particular, I will as the DM always try & find a way of fudging the dice/the events so that it allows the player to still be involved, dosed up maybe on painkillers, wound coated in field dressings &/or Realskinn, combat drugs, whatever to get them to the next checkpoint so a bit of down time can be incurred without detriment to the game.

Man, that's a lot of stuff!
I'd have to spend a free afternoon reading it, it seems...
Have a sit down on a rainy Sunday afternoon, have a coffee & maybe a biscuit or two & relax . . .

So who regularly kills the player characters in the games they run or pushes them to the edge but always leaves a back door. I guess I'm in the latter camp, I've been gaming for 30+ years & running games, AD&D, Runequest, Cyberpunk, Bushido etc for around 28 of those, in all that I've I've only ever killed two, yes two, player characters, well, strictly speaking I didn't kill either.
In the first case, it was my first game ever running the game & one character, being run by a very experienced player, walked into a lion & it ripped him apart whilst al his comrades were asleep, the second was a cyberpunk character who got shot in a case of mistaken identity by a fellow player character, so really neither was my fault guv' it was all a genuine mistake.
At the other end of the scale there's a dude I know in whose game I played weekly for around four months, it was virtually guaranteed they somebody in the group would die, sometimes more than once in a session, my first character in his game, AD&D, lasted around 11 minutes, longer than it took to role him up.
Don't get me wrong, I like to hurt my players, I like to make them bleed, I'm just quite keen they get up again so I can make them hurt some more.
Mod's - Apologies, put this on the online gaming bit in error, oops
